Empath Health is the parent company of Tidewell Hospice, Suncoast Hospice and Hillsborough Suncoast Hospice. HMC CEO Rick Bourne said the partnership with Empath will allow HMC to better adapt to the evolving payments landscape, including the demonstration of a value-based insurance design model (VBID).
“We started seeing the writing on the wall regarding Medicare Advantage unbundling and the future of VBID, and we knew we had to be prepared. Our county has a Medicare Advantage penetration rate of 53 percent,” Bourne told Hospice News. “We started conversations along the way and we talked to several other organizations. But we quickly realized that Empath is very mission-driven, which is very similar to the culture of Marion County Hospice.”
According to Bourne, HMC’s patient census currently hovers around 390, though it could be as high as 400.

The deal includes two other firms in HMC’s portfolio, palliative care provider Carewell Supportive Care and durable medical equipment (DME) provider Accent Medical, which specializes in end-of-life care. HMC also operates four thrift stores to help fund its programs.
Empath and HMC are expected to integrate in early 2023. HMC will not change its name and will retain its existing executive team, including Bourne.
“Even though we’ve changed our name now to Empath, the reason Empath and then (legacy) Stratum came together was to really be an ongoing partnership, really looking for an opportunity for a relationship, to uphold the mission of not-for-profit, but as we say – effective end-of-life care,” Empath Health President Jonathan Fleiss told Hospice News. Told. “We are truly committed to growth, not just for scale, but for a number of market opportunities, ensuring that we continue our mission.”
Empath Health merged with Stratum Health System in 2020 to provide a complete continuum of home and community care. The company provides hospice, home health, primary care, palliative care, PACE, HIV and sexual health care and adult day services.

This relationship will help HMC develop business lines beyond hospice, including home health care and palliative care in the near future, and possibly primary care and PACE in the future. Meanwhile, Empath will get a hospice home in Marion County, where she already has a home health agency. The subsidiary will expand its DME business through the integration of Accent Medical.
“We actually have a pretty broad range of DME services and last year expanded into the Tidewell Hospice area. We’ve been offering this at EmPath for the past two decades,” EmPath CEO Rafael Ciullo told Hospice News. “So it’s a service we believe in. Of course, we want to offer that service across our integrated care network, and it’s building on our pharmacy service, which we’ll expand and deliver together.”

Team assistant Taisha Atkins, who worked at Marion County Hospice for 10 years and was previously a CNA at a local nursing facility, said she appreciates hospice staff’s “one-on-one” approach with patients.
“I have good support from my manager and colleagues. We have social workers (who) can talk to staff if needed (and) we pull together. Our CEO has an open door policy (and) provide good communication,” he said. Email.
Mary “MJ” Schwartz is a registered nurse and clinical instructor in training who has been with Marion County Hospice since 2014. Hospice care is “patient centric,” he said.
Schwartz has learned a lot about working with hospice patients and feels that hospice in Marion County is “real.”

Lorna Rembert is an RN-BC with the Marion County Hospice/Nursing Home team working on patient care in the field. She worked in administration before preferring to work directly with patients.
“I work for hospice because I believe in and am committed to our mission: to provide exceptional, compassionate hospice care. My father is at the end of his life, in my home and cared for by the team I now work with. I can be his daughter, not his caregiver,” Rembert wrote in part in the email.
